This work is realized under the tutelage of Dr Catherine Delguste, who wrote a communication at the end of this email, and under the European convention of work for graduation. To achieve this, I need to contact a maximum of rehabilitation centers in Europe. To gather a maximum of information in a minimum time and inconvenience, I prepared a survey to be fulfilled by your team. It is an Adobe Formulary PDF, to be preferably opened with Adobe Acrobat Reader (free), so you can type your answers to the questions directly on the document and send it back by email easily. It is also possible to print the document and send it fulfilled by post at the adress specified at the end of this e-mail. If fulfilling this formulary was still too tedious to do, it is absolutely possible to agree on a telephonic interview or a videoconference (Skype), where I'll type myself your answers. I personally commit myself to not reveal nominatively any information gathered, but to treat it anonymously in order to have a global picture of all the centers in Europe.
