I think what you're seeing is that some genes appear twice in the
results, once with the Refseq ID and once without it. This is because
the Refseq ID is a transcript-level annotation, and so you are seeing
results per transcript, but without the transcript ID they appear to be
duplicate but with parts missing. If you turn on the Ensembl transcript
ID attribute you'll see what I mean.

Your first example, ENSG00000127580, is exactly like that.

In the cases where you say they are missing, I think you'll find that if
you search the results you'll find that there is another record
elsewhere in the file which has the Refseq ID present.
