Great article.  The early Soviet airliners, though considered decent quality, were rarely built with safety or efficiency in mind, and quality was only a concern if the higher-ups said anything.  This resulted in a much larger number of crashes than those experienced with American airliners, which were built in-house at the time before outsourcing.  Many jets, such as the DC-9, were sold to socialist countries like Yugoslavia.  The DC-9 proved itself to be an incredible investment and worked decades longer than planned.  The problem therefore seems to be connected with incentives for the elite, both Soviet and now American, to do anything it takes to gain status.  If you can do something, anything, on a lie to prove to the elite that you succeeded, you’ll lie.  Meritocracy on both sides of the pond has always created a sense of artificiality that disincentivizes workers to be truthful, so most will keep their mouths shut to save themselves and pay their bills or get ahead.  Creating new systems of incentives based on truthfulness and less on performance metrics might help.
