Took a number of fixes, from getting the error propagated correctly up from the
UserManager (20556, 20559), to propagating the error through the jsp (20557,
20558), to fixing the Javascript to look for failures correctly. Also did some
incidental styling improvements.
I believe each piece of this fix is fairly straightforward and I looked for
dependencies from other parts of the code that might be affected. The fix
looks quite self-contained to me, and I don't believe there will be any
unintended consequences.
