It has been some time that I've used iptables, but I'll try to help.

Let me recap to make sure that I've understood your intention
correctly:

You have your MASQ-Box, on which you have one Interface for internal,
and two interfaces for external traffic. One of those extrnal
interfaces
is used to handle all the "regular" IP-traffic such as the
web-browsing
of the clients in your private network. The other one is supposed to
work as a "Front-Door" to another machine that is also inside your
private network, so that a computer that talks to the 2nd external
IP-Adress really talks to your computer inside your network.

If I understood you correctly, you can write some rules for this.
I do not know them right now, but I can look them up.
