Usually, public safety units have no problem handling day to day
garden variety incidents. No first responders gets hurt - maybe some
property gets saved - maybe some healing is done - maybe some pain is
alleviated - maybe an argument is settled - maybe a life is saved.

Then the big one hits. And always - something goes wrong. One detail
is omitted. One step of the procedure is forgotten. More lives are
lost than should be. More property is lost than should be.

Lets call this "the choke syndrome".

Whats your SOP for handling the Choke Syndrome?
