there seems to be 3 levels of availability of surgeons and staff

1. dedicated to trauma and in the hospital 24/7

2. in the hospital 24/7 but not dedicated to trauma care 24/7

3. not in the hospital 24/7 - available within 30 minutes

this might be based on American Trauma Society guidelines - which
might be different than American College of Surgeons guidelines -
which might be different than Maryland guidelines - which might be
different than hundreds of other guidelines
