See section 5, especially 5.1, for the OPF formulation that shows exactly how 
reactive power enters the optimization problem. Practically, if you want to put 
costs on Qg, you add another ng rows to the gencost matrix (see first footnote 
on Table B-4 on gencost).

I'm not sure I understand the question. MATPOWER includes (in runmarket) the 
ability to include reactive power offers which are treated in a similar way to 
active power offers, though the two markets are coupled through the network 
[1]. For a number of reasons [2], I do not believe that it makes sense to 
handle reactive power in this kind of market in real-world applications.
