We see it in production maybe 5% of the time, which is pretty high.
Essentially, the CA is producing media that ffmpeg can't seem to read
(after ffmpeg has converted the media).

Quick fix is to just shut off break-on-error, but of course this isn't
ideal.  If it's needed in the short term I think we should shut it off
by default. Comments welcome,
