What xslt processor are you using? Sounds like what happens when the processor screws up. I believe there are problems using the Xalan 2.2.D14 development release.
Maverick 2.0 has much better handling of this error, and in any case, I'm going to make sure that the Xalan 2.2 works. I'm putting together the opt-domify package now. Try this url: http://blah/friendbok/signup.m?mavMaxTransforms=0 and see if that prints out the XML. This causes the domified XML to be serialized directly without processing.
