This looks like it.
PID 754482 does not exist (any more?).
It looks like it's stuck waiting for I/O or something?
Should I be killing 4 or T4, by the way, I've tried cancelling and killing
both, but it's still there in a 'sh ta'.
Any other suggestions?

your Utility Task is already in use by a DBM client. "dbmcli -d ... -u
...,... db_cons sh ta" gives you a list of the database tasks, one is marked
as Utility. Find out which DBM client is using it and end that client or the
connection of that client to the database ("dbmcli ... db_cons cancel
<task_index>").
