Mac? Windows? Linux? DynaPDF can give you a 300 dpi rendering of the page which you can draw in a graphics context for a printer. Not perfect, but works. on Mac OS X you can check our examples about printing. There is one which draws PDF pages on PDF context using CoreGraphics which preserves the vector graphics.
