Scrying with 'lsof' has revealed two ways in which program output gets lost. One seems to involve dangling pipes. Pressing F11 (Process Block) with no block defined tickles this bug on both my Linux boxes.
The other way involves mc closing its stderr (file descriptor 2). How to provoke this is still unknown. A simple manual way to detect if this has struck already is to prepare a shell script that writes a bit of text to both stdout and stderr and run it by selecting it in a pane and pressing Enter.
