I have not found anything as of late that meets all of those requirements.

Mostly I have found that if I want something asthetic and sounds good, it
is going to require a external amp.

I have been using either Innovox or James Loudspeaker brands for these.

The one that I had that had the integrated amp stuck 6" off the wall past
the monitor and had front display buttons we had to cover up.

To run these speakers I have been using a Pyle PTA4 stereo 120W amp. It
just fits behind a monitor and sounds "decent enough".
