My gut immediately screams storage driver. What if you hand built the machine? Does it work with the out of the box Server 2012 R2 media? What drivers are you adding through SCCM? Can you try it without any 3rd party drivers?

Does anyone have any experience imaging this particular machine or similar or can suggest something to try?
