1. Check your logs
2. Verify the inputs by exporting the variables with something like this:

My workaround was modifying the UDI wizard so a different value is set
instead of the usual defaults, but I made it so it's read only, and set the
actual credentials within the task sequence under Apply Network Settings,
which saves the password in encrypted form in SQL.

Your results may vary, I was able to use a service account for this project
instead of having the techs input their own.Worst case scenario write your
own prompt for these credentials and set them in the backend. Seems like
there's an issue with the UDI for this page.
