Change subject: force type=submit for buttons; provide placeholder attribute

force type=submit for buttons; provide placeholder attribute
1. The type attribute must be forced to "submit" on button elements, else IR
might interpret it as "button".
2. Provide a placeholder option for {{#forminput:...}}. This of course only
works if $wgHtml5 is true.
