Clean InitialiseSettings.php
Comments used a variety of free form format before the " // Txxxxx"
standard. We provide more uniformity in legacy request comments and
upkeep URLs to improve legibility.
Last remaining Bugzilla references to Phabricator tasks are converted too.
Finally, this change fix some spacing issues: vertical align is frowned
upon, but when used, alignment should use spaces. Tabs are for indent.
