Hi Roberta,
We are in the 5 to 6 day range depending on the account type (Journal).
We have a very detailed Journal set up..  specific to the nth degree, 
although this is 
not necessary at all for anything else.  Almost to the point where an 
individual clinic 
secretary feels s/he owns it, and it better be right and COMPLETE.
 
When we started Meditech
in '89, most charges were entered by users directly into BAR Batches. 
Over time we are now reading automatic batches from most ancillaries.  But 
there are 
holdouts that still won't use OE etc, and exception reporting from our 
PICIS-OR system 
is not quite good enough across all 3 sites to turn on automatic batch 
creation for OR 
MM Charges.... yet.
 
Then there are those odd situations that haven't been addressed like the 
special order bed
or Prosthetics which are still entered in BAR Batches by the users.
 
At this point dropping 1 day on getting the claim out and the resulting 
increase in cash 
flow  is worth the possible expense of lost charges.  But we do have a 
select few (Nursing) 
staff/trainers who can enter a late charge after the BAR Cutoff date. They 
of course work to find
ways to ensure that that a particular area either automates or improves 
their systems.
(They hate being called to enter these charges!)
 
User Departments regularly get Late charge reports and Fiscal Service will 
question them 
if the volume becomes "problematic".  There's also a cultural 
recognization that if the 
supply/procedure/test wasn't documented in the system/on the claim, then 
we mght have an 
audit (Fiscal or Medical) problem.
 
Ask if ABS wants to unfinalize an Abstract for Late Charges.  They can 
turn this off.
Unfinalizing can be a problem for getting a claim out on time depending on 
the type of 
charge, which could conceivably change reimbursement (DRG, APC, Coding).
 
All of us should have a goal (my Opinion) of being able to generate a 
complete, valid claim
on the day after the patient leaves. And present a complete list of 
itemized charges (less
Abstracting info) as they walk out the door.

thanks for kindly answering my question.  Most of my responses point to
this.
I think the real problem lies with trying to drop the bills in 5 days, but
then having straggler charges come across after that.  BAR doesn't want
any charges to cross after that point, even to the rejection report.  If
the charge is insignificant, they want to ignore it, but if the charge is
significant, they want the module that owns the charge to do a manual
process to have the charge put into the system.
 
How does your site deal with this issue?

In the BAR Journal Dictionary, BAR can set these 2 entries to restrict the
window on
charge entry.
 
Allow Charges For Service Dates  X  Days Prior To Service (Admission)
                                 Y  Days After Service (Discharge)

Our BAR wants to prevent charges from crossing to them automatically after
5 days.  We can prevent this in OE with a parameter setting, but LAB, RAD,
PHA, OR charges can cross whenever they are entered.  BAR wants this to be
a manual process after the 5 days.  Anybody else dealing with late charges
in an innovative way?
