Someone posted just a few days ago wanting
to know what information you have in your Medication Reconciliation form. If
someone can tell me what the subject name was, I could look it up in the
Iatrics archives so you don’t have to resend your responses again. For
those who wouldn’t mind sharing it again, I will surely appreciate it.
