Meditech has a DDP proposal, POM DDP 4051, for a Device Site
Restriction, and has had the proposal for some years now, that will
resolve this for us and all other hospitals that are having this same
problem noted below.

The DDP establishes how a site is associated to a patient location in
POM when one facility has multiple sites.

If other facilities fall under this same dilemma, request Meditech to
move this Out of Sequence and get it into 5.5.

All our in patient units are currently POM enabled and providers are
entering orders for their patients.  This is great.  Sometimes they even
do this from their offices when they get called about an in patient.
This also is the way it is supposed to work.

Here is the problem.  We have multiple lab sites.  These are located in
the clinics that we own. Everything is on our WAN so their devices work
just like they were located in our building.   Devices in the clinics
are managed in the lab device dictionary to their particular site.  If
an order is entered their the system sees the specimen as being at that
site.  Certain tests like CBC's and UA's are performed at several of our
sites.  If the provider enters the order the order for a test that is
performed at the lab at the ordering site and also at the ML site, the
system sees the specimen at the clinic site.  If you use the change site
routine to try to move the specimen to the ML site, you get a pop up
that tells you that there are no tests that need to be at ML.  If you
try to enter results, it tells you that there are no eligible results.
Your options are to switch to the ordering site and enter results as if
you were doing the test at that site, attaching a site footnote
incorrectly to the report, or to cancel and reorder at the ML site.
Neither idea is very appealing.  The system is working as designed, I
feel we need to fix the system.

I suggest that we need to modify the lab site routines so that patient
locations can be linked to a specific site.  If the patient is
registered to an inpatient location, the lab site would always be ML.
If a patient's location is one of our clinics, they would always be at
the site for that clinic.

I could probably remanage all devices in the clinics to ML except for
those that the lab staff uses.  This is at best a temporary work around
because Ambulatory Order Management is coming and clinic orders will be
entered from the desk top.

I am looking for help on how to carefully define this to Meditech so
that they can program it so that it works right.  Please send me ideas
on how you think this should look.  My vision is to add fields to the
SITE dictionary that allows us to list entries from the MIS location
dictionary and indicate that orders for these locations are always at a
particular site.  Other ideas will help refine this.
