If the reflex is based on medical criteria (like PLT of 50 or WBC of
<1.5 or patient age less than 2 years old for example), and not to
confirm something on the automated count, then you could charge for the
reflex.
However, if the reflex is to confirm something on the automated count -
an R code or system message, then you can't charge for the reflex
because you are validating the quality of results, not providing
definitive clinical information.

Hi all,
I know this subject was bounced around the L several years ago, but I
wanted to get some input again.
I realize that when a physician orders a CBC with manual differential
that you can charge an 85027 and an 85007.
My question is this. If you reflex the manual differential, are you
still charging for it, and if so, how do you justify this with the
Physician order if he/she only wrote the orders for a CBC? At my
previous site we were told that we could not do this. I was wondering
if any other sites were doing this. Is having a reflex justifiable if
the Medical staff has approved the reflex testing.
Any input on this would be greatly appreciated.
