We do the same thing. All of the Medispan dose range checking is basically 
turned off. I built dose checking for most of the chemo drugs but it is very 
iffy
as you cannot define - this is for a 2 week protocol, or this is for once every 
6 weeks. I also built dosing for some drugs like acetaminophen - for adults
and pedis but I sort of le that fall to the wayside.  I built flags for some 
drugs to suggest doses based on creatinine clearances for the pharmacists, but
now the MD's are using GFR's and everyone is confused. I think I'll just go 
beat my head on a rock.

Jane - I basically built my own dose ranges rather than use FirstDataBanks in 
their present form. Also, with chemo, there is so much variation according to
protocol that we have 2 pharmacists manually check the dosages against the 
appropriate protocol. All that my dose ranges do is set a max dose, which could
be way to high for a particular protocol.

 Hi there,


 We have Micromedix formulary and do not receive any dose checking info for 
Chemo Drugs.  How are other Pharmacies handling this?  Ie. Are pharmacists
 entering the information into the dictionary, are techs or IS support 
responsible, is there a web site (ie Micromedix – to obtain the information), 
how
 often do you check, etc., etc.
