I Patti,
I've only done user-defined demo labels and report forms but I think you
may need to define the UDF in the Exam Type Dictionary.   On page two
there is a column for formats next to each type of print option.  I had to
put the report form on this page and I had to put the demo label form in
RAD defaults.  Hope that helps.
