We had automated our observation room charges close to two years ago.
While I have to admit there are some bugs, for the most part the debit and
credit of the inpatient room charge to and from observation works fairly
well. The accounts that we have issues with are the ones that were changes
well after the patient was discharged.
In our case, we have the reverse scenario in which our patients
start out as inpatient and the documentation only justifies the admission as
an observation. We just updated from Magic 4.9 to 5.5sr1 and I did "break"
the process in TEST where I had more credits for observation than actual
charges that hit the account; but so far our live accounts have been
accurate. Check to make sure that your system is current as there were
several DTS addressing this issue.

What are you all doing in the following scenario:
Patient is registered as an INo (OBS)
Midnight occurs and OBS hours/charges are sent to BAR
It is later determined that the patient should have been IN from the start
so the admit is backdated
The system sends a room charge over, but does not back off/credit the OBS
hours
If you have a solution please share (other than getting the PT status right
in the first place :> ).
MT's response is to hold these accounts in BAR and manually remove the
charges. We have been doing this but are tired of the manual work. MT has
been sitting on a development request for years to fix this issue. If you
are also frustrated with this broken/manual process, please contact your ADM
specialist and ask them to fix it.
