Attached is a keyboard template that was sent to the L in 2004 (yes, I
save things!) from Tansy Caines at Northeast Medical in Concord, NC. I
don't know if she's still here, but we have certainly used this one.
Someone in her facility did a good day's work to create this in Excel.
