the IHFS (formerly Illinois Medicaid) HFS3797 form that is to be used for
billing for Medicare Crossover professional component charges and is required
as of 04/01/06? This form is replacing the DPA 2360 Health Insurance
Claim form and HCFA 1500 Insurance claim form for submission of claims. Please
