Hi,

The Care Plan dictionary here is empty.  We create custom plans as Assessments are documented.  Queries are linked to patient problems (diagnosis) via the Query Link dictionary.  The new problems are filed on the POC automatically after the assessment is filed.  All clinical departments are taught how to add problems directly to the POC, also.

Hi,

My facility is in the middle of revising our plans of care.  We would like to make the Care Plans more user friendly and appropriate.  Currently we have several interventions on many problems that are not being used by the staff to document patient care.   Is there anyone that may offer advice or give an example of what your facilities Plans of Care are like?
