We use our interventions and schedule them. For anything that they need to be reminded of that they will chart in an intervention we have them set the directions to those specific interventions. For things such as peak and troughs and such that you don't necessarily chart we created an intervention called "REMINDERS" and taught them to put in the EDIT TEXT area exactly what they need to do. On the Nitro patch our Pharmacy agreed to enter REMOVE PATCH like a med and it is scheduled for 2100 and they document that they removed it. For all other patches that you remove and reapply they are just in as a med. We have recently added a few other specific reminders: REMINDER - glucose bedside test REMINDER - H20 enteral flush REMINDER - IV/MED REMINDER - lab draws REMINDER - pacemaker checks REMINDER - X-RAY One example of use we came up with during live was a pt that needed to have a specific med given 20 minutes before a procedure the next a.m. We had the CICU nurse enter in a reminder for approximately 30 minutes before the procedure and that came up on the status board for her to see. This works great for reassessing pain relief and a variety of situations. One question that keeps plaguing (no, this is just the one today) is how to notify the nurse of all the stuff they used to pencil-in on their paper MARs. Stuff like nitro patches have to come off at a certain time, then go back on. Stuff like remembering to draw vancomycin peaks and troughs when the dosing isn't every x hours. Stuff like documenting giving units of blood. Stuff that just can't be ordered by pharmacy and land on the MAR. Are there any creative solutions for this kind of thing out there? I'll take truly mundane solutions that work, too! Thanks for all help, past, present and future.
