I just remembered one really important thing about the zcus formats for our
RAD forms.  There was one field that called a line of programming that was
too long for us to translate here and the report would kick us out of
Meditech completely.  That field was the one for the transcribed date and
time.  The VAL statement for that field had to be broken up with part of it
put on a line check.
This is a problem we have had with a couple of reports that Meditech can
translate but I could not.  This has been even more problematic since I
began using NUI.
  If you are being crashed completely out of Meditech during your
translation, you may want to ask your RADRW consultant if this could be the
problem.

When you moved to 5.4, did Meditech not provide you with custom report
formats (called udf) in your RADRW module.  Meditech gave us zcus formats
for our REPORT, ADDENDUM and IMPRESSION that we have since customized even
more and set up as our report format in the RAD module.  These reports are
named exactly the same as yours with the exception of the zcus,
RADRW.REPORT.zcus.rad.udf.report.format.2 ,
RADRW.ADDENDA..zcus.rad.udf.addendum.format.2 and so on.
