Paul, Are you C/S and if so would you mind sharing the report? -----Original Message----- From: [EMAIL PROTECTED] [mailto:[EMAIL PROTECTED] Behalf Of Paula Taylor Sent: Friday, April 07, 2006 11:14 AM To: 'Medeiros, Amy'; [email protected] Subject: RE: [MEDITECH-L] Medication Reconciliation at Discharge Iatric did a report for us that pulls home meds from the admission assessment and current meds (while in the hospital). It's in a format that has a check box "yes" or "no" for the physician to order which meds the pt is to be discharged with. It also has a section that allows the doc to add any new meds that he wants to include. It pulls everything by trade name so ECFs really like it. -----Original Message----- From: [EMAIL PROTECTED] [mailto:[EMAIL PROTECTED] Behalf Of Medeiros, Amy Sent: Friday, April 07, 2006 6:17 AM To: [email protected] Subject: [MEDITECH-L] Medication Reconciliation at Discharge All messages should be posted in plain text. Should you receive any info that is not on the "L", please be sure to post or contact me. Thanks Amy J. Medeiros ED Core Team (386)226-4509 [EMAIL PROTECTED] I'm just wondering what everyone is using for Medication Reconciliation at Discharge? Especially when it comes to the discharge instructions...we would like to be able to pull in both the home meds that the nurse enters and any ordered meds from Pharmacy...then allow the nurse to edit whether they've been discontinued or not at discharge....does anyone do this? Is it possible? If not what is your hospital's process. We are a Magic 5.4 hospital.
