Our current practice is to document the titration of medications (drips) in
NUR via CDS's attached to interventions. We will be going live with EMAR
soon and the question has arisen re: how to continue this documentation.
One school of thought is to continue as we currently practice and document rate
changes in NUR and document on the EMAR only when there is a new bag hung.
The other school of thought is to attach CDS's to the various drips and have
nursing document rate changes on the EMAR. The problem is that many of
these drips (such as Dopamine) are high alert drugs and will require
a co-signature.
How have you dealt with this issue? I am concerned that if we
document rate changes in the EMAR that it will result in a nursing being very
dissatisfied as they will need to find someone to co-sign every time they change
the rate.
