We have EMAR up, but not BMV, so I don't know how this will work for BMV. For the EMAR, what you are describing is a range order - Our pharmacists put it in the system as a zero dose, with the parameters in the label comments. Our physicians are supposed to write the order with the parameters ( 2mg for mild pain, 4mg for moderate, 8mg for severe) and we have a pain policy that defines what mild, moderate, and severe pain is on the pain scale (ie. numeric scale "8" constitutes severe pain, so nurse picks corresponding dose to give). Nurse documents amount given using ADJUST DOSE routine and the attached assessement has a query to document the pain score used to determine how much to give. We are in the process of building for CPOE and we are building the parameters for the MD's into the order strings. Will be lovely when PRN reason fields work in POM!
