Our facility was warned that you needed "proof" of interdisciplinary team conferences...and, that you may need to pull it up hurriedly for JCAHO. With that in mind, we created a NOTE type of PCC, patient care conference. We use that note type anytime there is an interdisciplinary meeting or communication between the disciplines.
