I see by searching the archives, that lots of people have had problems with zero dose orders.  We have tried to minimize the orders we enter with zero dose.  Morphine 2 to 4 mg IV q4h prn used to be entered as zero dose, now we enter two fixed dose orders for 2 and 4 mg.
 
But, we still enter zero dose orders for creams, inhalers, eye drops, sliding scale insulins, etc.  Does anyone have any better idea?  Is anyone documenting with 1 application, 1 puff, etc. on the eMar?
 
If so, how does it work for you?  How did you set it up.
 
With using zero dose orders, we have found a lot of documenting errors from the nurse entering the wrong dose.
