I don't know if you had a reply yet, or if this is what you are looking for but we made a Path Consult order with specific fields that have to be filled in before filing - this paper prints at the ordering source and comes with the specimens - page 1 has questions ie.. procedure performed, diagnosis, frozen section etc.. page 2 has 6 spaces for tissues (ie.. A,B,C,D,E) that are filled in to correspond with the actual specimens - it is filed and the "order" is then on file in MediTech capturing the initials of the ordering person. We then enter the specimen into the PTH module. Anita Sutherland.
