We are in the process of writing a policy (I've asked that the policy be written before we start asking the staff to document 1st dose), the policy is a very generic one that calls for 30min and 60min assessments, that is all our pharmacy manager wants, he doesn't want to get into this drug has to be monitored for 12 hrs, this for 1 week, etc. He wants it general and easy so that it will be done. I have created a cds in NUR for the nurses to document on. Our acute care manager wanted it on the shift assessment, but I am saying "no". Say you do rounds on your patient at 8am, Dr.Jones comes up at 9am and writes orders for a new med, pharmacy gets the orders and profiles them by 10am, you administer the med at 10:30am, you sit down to document your 8am assessment at 12noon because you don't like to take a "cow" in the patient room while you do your assessment. It looks kinda strange that you document at 12:30 that at 8am you did and assessment and were predicting that you would give a new med and be able to document 30 & 60 min assessments based on your 8am time stamp. None of the nurses want it on the shift assessment, they want it on it's own screen, they feel they will be better able to document it on it's own screen. Now here's where administration doesn't like me, what about ancillary departments that administer drugs, IV dye, Creams, lotions, injections, glucose tolerance testing, etc..... how are they supposed to follow this rule????? Who is going to "police" the staff to make sure they are following the rule?
Any ideas?????????
Is there a way to set-up emar with a rule that the first time a drug is administered it will flag the nurse????
We will be going for emar and bmv training in Aug. '06.

Are there any facilities out there that have found a solution for the
national standard regarding Nursing documentation of first dose
administering of meds?  Current policies and procedures would be great
to get and even exactly how the process works from the Pharmacy to
Nursing.  We are trying to come up with a solution to this but are
running into problems with the fact that depending on the med
administered there could be a time frame of 4 hours before there could
be adverse reactions all the way up to 12 hours.  Any information would
be helpful.....Thanks,
