Sharon, before we were live with barcoded medication administration, we relied 
on the nurses to report their medication errors through incident reports. That 
is still the expectation when an error occurs. We didn't have them fill out an 
incident report for a near miss before, and we don't now. We do encourage the 
nurses to report near misses to me if they think there is a system or process 
error so we can address it and fix the problem.
