We have accomplished this by having one order in Meditech called CT
Thorax/Abd/Pelvis, and it goes to PACS as one order.  In the meditech
dictionaries, there is a billing code attached to the procedure for the
CT Thorax, and then another billing code attached via the materials
dictionary, which is attached to the procedure, for the abd/pelvis.
This enables us to have one order to PACS, and explodes on the billing
side in Meditech.

We use Fuji but have not moved to 5.5 yet. We do not use combo codes
when 
We order, we have everything indivualized with queries that designate
Right,
left, bilateral 
Where appropriate. For example, we have Chest xray 1 view, Chest xray 2
views etc. 
CT Brain, CT Head , CT Shoulder etc... Each order has its own unique
number 
We had MEDITECH create and put on our requisitions and is also sent to
Fuji.
Fuji calls it Accession number. Basically it is the Date , the order
number
, and the 
Category of the test for each order. For example an Xray of Chest 2
views
,which 
Is first order of today would have accession number of XR20060615-0001 .
An
CT of the 
Chest the third order today would be CT20060615-0001. 

If there are 3 images taken together , they all have individual
accession
numbers, 
Fuji takes the messages that come from Synapse and sends it back to
MEDITECH
in all 
Three accession numbers. 

Also we had to change a few of our categories in MEDITECH because the
accession number can not
Be longer that 16 characters. It wasn't that big of a deal but it was
some
effort. 
For example we had WTWMAMMO as a category , we had to change that to WTM
so
the 
Accession number would be WTM20060615-0020 for the 20th mammo ordered
today
. 

We had to take the query responses for R, L or B and append them to the
test
Mnenonics that are sent to Fuji in the interface string. MEDITECH did
this
for us. 
Fuji gets it and then displays the appropriate word for the radiologist
on
the
Fuji side so they know what they are looking at before they go into the
image. 
For example if the CT example above has mnemonic CWWOC and the query was
answered 
As R ( for Right ) , the system would send CT.CWWOC^R^RIGHT to Fuji. 

Then the radiologist would see Chest xray 2 views RIGHT on fuji side.
This
is just
An example I made up, this probably isnt a great example of right or
left
but you 
Get the idea. 

We also had MEDITECH enter in a global username and password that is
used
only when 
A user is in EMR and clicks on an Image in Imaging. MEDITECH passes the
global username
And password behind the scenes to the Synapse server and the image
appears.
We did this
So the users would not all need access to Synapse where they can see all
patient xrays.
Instead if they use EMR they only see the one patient views. 

A problem we have here is the technicians who take the images frequently
do
not use
The correct accession number . They cancel or the test is cancelled
after
they shoot
The image. When that happens the MEDITECH can not find the image because
it
uses the 
Accession number it looks like there is no xray in EMR. This is because
we
have our
System configured to send the image to EMR and ITS from Fuji as soon as
the
technician
Sends the image from their xray machines. There is an option to send it
to
MEDITECH 
When the dictator marks the image in Fuji as dictated. I would suggest
using
that one. 
Originally our ER complained that they wanted to see the images in EMR
as
soon as they
Were shot before the radiologist dictated. That sounds appropriate , but
in
reality, 
They doctors who do the diagnosising in the ER, don't use EMR they use
Synapse directly
So it doesn't matter to them.
