To answer your questions (listed below):

b. - Yes, we require a double check of insulin and
heparin.

c. - Yes, double check must be documented by a nurse's
initial and time on the MAR.  

d. - We currently are using a paper MAR and for
insulin and heparin, under LABEL COMMENTS we've
entered fields nurses must sign for the double check
signature, including date and time.  BTW .. we are in
testing for BMV, and that system is nicely set up to
accept verification from a second nurse.
