Many facilities use the first 2-3 characters of the user's last name (in some cases, maybe even more characters) as a basis for the user's username (or userID or user mnemonic, as referred to in some applications/systems).
 
My question is in regards to any formal policy that any organization has in place, defining if and when usernames can be changed and how?? Specifically, my concern has to do with username-based audit trail reports. If usernames are allowed to be changed for any of a variety of reasons, how do these impact these audit trail reports? In other words, if you allow and make these kind of changes, how are you keeping track of them so you know a certain user may have more than one username during a specific period of time??
 
If you have a formal policy regarding user name changes, I'd appreciate it if you would also email me a soft copy of that as well. Thanks!
