I am having a similar issue, though mine involves the rad number versious the 
MR#. Our Radiology director chose to use the Rad# as the unique ID, the rad 
number is not assigned until and rad order is done, what to do? We need to 
change to the MR#, but what about all the old accounts that input manually 
before the interface goes live, they all have Rad# as unique ID.

I am forwarding this question on behalf of our PACS administrator.  This 
problem stems from a couple of things: 1) We do not assign medical record 
numbers until new patients are registered for the first time (as opposed to 
assigning the MR# at the PRE status) and 2) Exams are pre-ordered at the time 
the patient is scheduled (this is the point that Meditech sends the exam record 
to PACS).

For accounts with a MR#, records sent to PACS are identified by the MR# and 
assigned to a patient folder identified by this number.  But when the account 
is in a PRE status, the PACS record/folder is created with the patient account 
number as the patient identifier.  When these patients are registered on the 
day of their exam, a MR# is assigned; however, the record identifier on the 
PACS side is not updated.  We currently have to catch and manually fix these 
within the PACS system once a MR# is assigned.  If we do not catch these, the 
patient's subsequent exams are assigned to a new folder (identified by the MR#) 
as the patient will have a MR# when they come in for additional imaging.  This 
is causing problems for physicians when they attempt to view the images.

Has anyone else had this issue with McKesson PACS?  From what I understand, 
McKesson has not been very helpful in finding a resolution to this issue.  We 
do not have this problem with other systems to which we interface because they 
have logic on their side to hold records somewhere until accounts are in a 
registered status.
