In HCA we have a report that shows those staff that scan the med and then adjust the given time. While not a huge number, we do have nurses that scan (probably to avoid being asked why their scan rate is down) and back date/time it. Our managers are just starting to get this information so they can address each instance. It probably is a case of correct documentation, just that it should be a Full Document rather than a Scan Med. The place to document any meds that were given earlier is by using Full Document. I took Brenda's question to mean that the nurse could change the time that they gave the medication to an earlier time than they actually gave it so it didn't look like they were late giving the med when they really were. We have been up with BMV since Nov and I have not seen any instances where someone has changed the time when they were scanning the med, even if they were brought into the full document screen. I have to admit, I haven't really looked either. Our superusers do express concerns about what some of the nurses are doing and changing the time when scanning has never been one of them. I would suspect any honest nurse wouldn't change the time. I have to believe that the vast majority of us are honest. Let you staff know that the time that they actually scan, document, etc is captured by the system as is any other documentation. I don't think that if they are documenting late, and actually gave the med earlier that you want the system to force them to document a wrong time. I think the light of eMAR shines brightly on many areas that have always been an issue, but are now brought to everyone's attention. I'm disappointed to find that even tho I have "Display Scan Med Defaults" option set to 'N' in my MAR dictionary, preventing users from changing the date/time initially when they SCAN MED, if they are outside the variance time, the system brings them into the FULL DOCUMENT routine, and DOES allow changing of the date/time. As it is, users are already starting to voice concerns about "now you'll be able to see all the times I give meds late". I'm concerned that this part of BMV will tempt them. I know this can be monitored via reports, but I'd sure like it if the option wasn't even there in the first place. Ideally I'd like to see a user ONLY be able to change the time of an administration via EDIT DOCUMENT, or by going directly into FULL DOCUMENT initally, but not from SCAN MED.
