Our purge parameters are not that high, however, I just recently left
MEDITECH and during my almost 7 year career there, I supported
ADM/ABS/MRI. There are a few reasons that MEDITECH advises against
raising this setting so high. The standard default for these ADM file
maintenance parameters is 30 days, however most health care
organizations do have these fields set to a higher value. Many sites had
this set to 90 days for the ALL SCH and NON SCH. Are you a MAGIC site?
If so, raising these settings can cause space issues on your system.
Also, the reason behind MEDITECH advising against having these settings
at such a high value (such as 7 years)is that your active account look
up will be very large. This will cause issues with response time as well
as patient look ups. When a user attempts to locate a patient by name
the system is going to need to search through seven years worth of
active accounts. This is problematic due to the time issue but also
leaves more room for user error. The more accounts that are accessible,
the more likely that a user can select the wrong account for the
patient. There are several fields that are stored within demo recall as
well that when updated, loop through and update ALL ACTIVE ADMISSION
accounts. The fields affected by a change to demo recall are: 

If the the following fields are updated in ADM/Demo Recall, it will
update the accounts in ADM:

The fields are as follows:  name, dob, age, sex, mothers name, other
name, social security number and patient address.

If the site has a guarantor billing system, then the guarantor address
will update all patients that share that guarantor SS#.


It is very possible that during a seven year time frame, a patient's
information could change, but the older information may have been
correct for the time of the initial registration. This may cause issues
for you with data integrity as well as possible billing errors (say for
example the Guarantor changes and all the active accounts were updated)
If you do pursue raising these settings, you should also be sure to
change your ADM toolbox facility specific parameter 'IGNORE PATIENTS
LAST EDIT DATE' to YES. If this parameter is set to NO then accounts
will NOT purge out of the system by the Discharge Date for RCR/IN/INo
accounts and the Service Date for CLI,POV,SDC,REF,ER accounts but rather
be purged off of the last time the account was edited. This too will
result in many more accounts remaining active in your system. 

In contrast, there are some parameters that MEDITECH does actually
recommend setting to a high value in order to keep data on the system
long enough to meet HIPPA compliancy. For example, in MRI, the
Correspondence purge should be set to 2290 days. This will store any
Release of Information requests for a time frame of 6 years as mandating
by HIPPA and also allow the user an extra time for the processing of the
request.
