We use the last 3 characters of the physicians last name and the first initial of his first name Example: John Doe MD mnemonic=DOEJ If we get another physician with the same 4 mnemonics we just go out another character on the first name and they will have a 5 digit mnemonic instead of a 4 digit. Example: Jason Doefner mnemonic would = DOEJA due to John Doe already having DOEJ. We tried to keep it consistent with how we assign user mnemonics. There is a field in the doctor dictionary that you can put their old numbers in for use on any custom reports you may write up. The field is labeled PROVIDER #. When we converted to Meditech we had the same issue of being used to having the physician have an assigned number, so we entered the number they were assigned in the older system into this field in Meditech and gave them a mnemonic in the format listed above. We eventually stopped using and assigned provider #'s but the field is there if you want to continue with the practice. Hope this helps.
