I can't point you to anything, but we have the same set up here.  It has
to do with Medicare.  If the patient has Medicare as their primary
insurance, and they return within 72 hours, they have to be given the
same account number for billing purposes.  Any other primary insurance
carrier, and the patient is given a new account.

I'll ask around to see if there is something official on that.

Can anyone tell me or point me to a regulation regarding a patient who
leaves one in-patient facility to another in-patient facility and then
returns to the original facility?  

We frequently have patients who leave our facility for a surgery at
another in-patient facility and then return to our facility.  Right
now
we treat these as LOA patients, rather than a discharge and new
admission, if they return to us within 3 days.  I am arguing that this
is not correct, but can not find any regulation that spells it out.
