Site Visit
In 1999, the Institute of
Medicine (IOM) published "To Err Is Human: Building a Safer Health System". The
IOM estimated that 44,000-98,000 deaths occur annually as a result of medical
error. These findings prompted the John Muir Health System to initiate a number
of patient safety initiatives, including implementation of bar code technology
in the medication use process. During the
site visit, presenters will share their BMV/On-Line MAR implementation
experiences from pre-implementation through full implementation in all of the
inpatient care areas, and review where they are today.

John Muir Medical Center,
Concord Campus is a 259-bed acute care facility that serves Contra Costa and
southern Solano counties. The medical center has long been recognized as a
pre-eminent center for cancer care and cardiac care, including open heart
surgery and interventional cardiology. Other areas of specialty include general
surgery, orthopedic, and neurology programs. John Muir Medical Center, Concord
Campus is accredited by the Joint Commission on Accreditation of Healthcare
Organizations (JCAHO), a national surveyor of quality patient
care.
Setting
the Pharmacy Standard for a Successful BMV/eMAR
Implementation
Patient
safety goals are consistently being brought to the forefront of healthcare. The
implementation of barcode technology has enabled many hospitals to achieve JCAHO
standards of such goals. This presentation will examine implementation of
BMV/eMAR in a Client/Server 5.5.2 environment with an emphasis in pharmacy
preparation. We will cover three stages of BMV/eMAR implementation:

This presentation
will discuss nursing training and implementation support for the BMV/eMAR
project at Methodist Health System in Dallas, Texas.
Topics will include:
• scheduling just-in-time training for over 1000 users
• standardized training content
• use of scripting and order sets for practice patients
• twelve week implementation schedule that included 20 nursing units at two hospitals
• to do list for the day before go-live, including user access, order acknowledgement
• BMV support team - 24 hour coverage for each unit’s go-live
• issues and workarounds after go-live

Bar code point-of-care
technology can significantly reduce medication errors. Stillwater Medical
Center chose to implement a bar code medication administration system other than
Meditech. This bedside medication safety system optimizes the nurse's role
in patient care and links to the hospital's existing Meditech information
system. The other vendor's system is a wireless nurse workflow tool that
provides bar code patient safety, bedside documentation, and clinical and
workflow alerts. This presentation describes Stillwater's selection of the
other vendor's system, the installation process and interfaces between the other
vendor's system and Meditech. An overview of Stillwater's specific project
considerations and implementation activities is provided, including the
development of ADT, financial and clinical interfaces to Stillwater's Meditech
system, along with detailed pre- and post-installation results.
