Usually our pharmacy will only load full dose tablets into the machines.
The order is entered as 1/2 tab etc...the nurse removes and breaks in half.
Some items like cytotec 50mcg and 25mcg (used a lot in OB) will be prepacked
with its own mnemonic and entered into the pyxis as such. Also, some odd
combinations will just be entered with inventory changed to "main" and sent
up in the cart fill. We do it all, depending on the circumstance.

Wondering if hospitals could comment on how they handle frequently used 1/2
tabs.  Is a separate pharmacy mnemonic created?  Or, do you use the
manufacture's size and then enter the correct dose.

Also, what is loaded in the dispensing machines?  The 1/2 tabs, or the full
tab and nurses break the tab?

Examples: 
Coumadin 0.5 MG  (Is an entry created for the 1/2 MG (1/2 tablet) and then
loaded into Pyxis/Omni), or is the 1 MG mnemonic used with a dose of 0.5 and
the nurse breaks the tablet.

Other examples/meds could be
Metoprolol 25 mg (before the 25s were available)
HCTZ 12.5 mg
Amiodarone 100 mg
