Title: NPR - Show Where Patient Was Located At Time Of Documentation

I'm kind of stumped on this one. Not sure if it is a easy fix or not.
From the DPM: NUR.PC.WORK
I am trying to get the patient's location at the time of charting. For example, patient was in ICU yesterday and they documented restraints and then today the patient is in PCU and they document restraints. I want something similar to this:
Date Patient Restraint Location
08/15/06 John Smith ICU
08/15/06 John Smith PCU
Any ideas would be appreciated!!!
