We do the same. We have custom formats for different Nursing Units, ICU, Med/Surg, Peds, Behavioral Health, ect. We even designed one for Respiratory Care that they use for their shift to shift report. Need to invest in highlighters though as the font is small. I kept working on them (with the emotional support of Gail from WaterTown) until the floors were at least not at my door every morning. You can add just about everything to the kardex and you don't have to depend on someone remembering to hand write the data on the paper kardex. We print our profiles before each shift. They originally had a manual process using a tri-fold Cardex that they would write information on. This then changed to a JOT sheet printed from Meditech that listed the patients on the units and their information from the system. We now have print profiles set up in Meditech and they were using a Cardex printed from a patient profile, but now are not happy with that. New issue is that they want me to create an NPR report to pull patient information, but be similar to their original tri-fold Cardex they could write on. I feel we are taking a step backwards and need to utilize the Print Patient Profile function to obtain all the orders in the system and ensure the nurses on the last shift actually entered orders as requested. I'm not sure which way we should go with this request from here. What are other hospitals doing to handle reports between nurses during shift change??? Thank you in advance for any and all responses.
